Microsoft has admitted the third service pack for XP (SP3) was missing a vital security patch.
The firm said a patch issued in November last year was not included in the pack, released in May.
It concerns the installation of Flash programmes, with the software giant issuing five patches for "vulnerabilities", with the patch marked "critical", the highest ranking.
SP3 was released with an out-of-date, old version of Flash, with Microsoft initially not answering questions on the error.
However, the firm has since come forward and said: "Any statement that Microsoft installs any versions of Flash Player with Windows XP SP3 is inaccurate."
With a spokesperson saying the update has a "hands off" approach to Flash by not including any so as to not disrupt the users existing system.
It is the latest in a long saga for the XP SP3, with the initial start date being delayed and then the software being released accidentally.
